Todd's belay loop was visibly damaged before it failed and if I remember correctly he was aware but though he could use it just a few more times.

Most belay loops that I have seen are actually two seperate slings sewn together so they are redundant.

No. You only belay off of one of them. You don't clip through both.

Obviously you can. They just didn't design it so you'd clip both when belaying.
